The chains figured out what independents were never shown

Walk into a PE-backed physio group and the clinical work looks a lot like yours. Same assessments, same exercises, often people you trained alongside. The difference is not on the treatment table. It is in the back office.

The groups grew by treating a clinic as a system. They knew their follow-up rate per clinician. They knew which site converted enquiries and which quietly leaked them. They could look at a Tuesday and tell you roughly what December held. Most independents ran on feel and found out in the year-end accounts.

It was never a talent gap

The three-room practice in Sheffield or Clapham is not losing ground to the group down the road because its physios are worse. It is losing because the group bought infrastructure the independent could never justify. A data hire. An ops manager. Software built for 40 sites and priced for 40 sites.

Consolidation didn't win on clinical skill. It won on knowing its own numbers.

That pricing was the whole story. The tools to run a clinic like a system existed for years. They just sat behind enterprise contracts and implementation fees that made no sense for a two-clinician practice. So the independents went without, and the gap widened one quiet quarter at a time.

The floor just moved

What changed is not the clinical work. It is that the infrastructure got unbundled. An AI receptionist that used to mean a call-handling contract now costs less than a part-time front desk. A performance dashboard that used to mean hiring an analyst now reads your existing Cliniko or WriteUpp data on its own.

We built StrydeOS in our own clinic in West Hampstead for exactly this reason. Not to beat the chains on scale, we can't. To hand the independent the same instruments the chains bought, at a price a three-room practice can actually run.

The independents were never short on skill. They were short on the tools. That part is fixable now.
